Parents/Guardians who have children in kindergarten through the fifth grade will need to complete a form to identify which option they are choosing for their child/ren. The form will be provided to families by their child’s school. You will also find the forms by choosing one of the links below.
You will need to complete a form for each of your children in kindergarten through the fifth grade no later than Friday, October 23, 2020.
